A non-working toll free number (800 / 866 /877 / 888) that has been disconnected within the last 90 days. Traditional toll free numbers and fax numbers okay.
Note: If Toll Free # is associated with a pager, cell phone, VoIP, or Voice Mail an additional charge may apply, however, you will be notified if # provided is not a traditional toll free number.

- The billing address on file prior to disconnect.
- Company or Account holders's name (if available)
